New Patient Registration

We welcome anyone living within our practice boundary (YO11, YO12 and YO13) to register with the practice. Please ask a member of the reception team for a registration pack.

Patients are registered with the practice and not with a specific doctor although when calling for an appointment you will be given an option to see any doctor working at the practice.

Newly registered patients will be asked to complete a health questionnaire and a registration form and they will then be able to access the services at the surgery.
